제2형 당뇨병 유무에 따른 DXA 기반 T-score와 Trabecular Bone Score의 비교: 후향적 단면연구

초록
This study investigated the relationship between Bone Mineral Density(BMD) and bone quality in patients with Type 2 Diabetes Mellitus(T2DM). A retrospective cross-sectional analysis was conducted on 300 Korean adults aged 50 years or older, including 150 patients with T2DM and 150 non-diabetic controls, who underwent dual-energy X-ray absorptiometry(DXA). BMD and Trabecular Bone Score(TBS) were analyzed using independent t-tests and Mann-Whitney U tests, chi-square tests, Analysis of Covariance(ANCOVA), and multiple linear regression. In ANCOVA adjusted for age, sex, and Body Mass Index(BMI), no significant difference in TBS was observed between the two groups(P=0.055). However, multiple linear regression analysis demonstrated that T2DM was a significant independent predictor of reduced TBS(P=0.018). Although T2DM patients showed relatively higher T-scores, TBS tended to be lower, indicating impaired trabecular microarchitecture. These findings suggest that BMD alone may not fully reflect bone quality in T2DM patients. In conclusion, TBS may serve as a complementary tool for assessing bone quality and improving fracture risk evaluation in patients with T2DM.
