Regional Disparities in Response Times for Out-of-Hospital Cardiac Arrest: An Urban and Rural Comparison

초록

The purpose of this study was to analyze the regional disparities in response times for out-of-hospital cardiac arrest, focusing on the differences between rural and urban areas by utilizing database from fire safety big data platform. This study analyzed 11,299 out-of-hospital cardiac arrest cases. Frequencies, means, standard deviations, while a student’s t-test was conducted to examine the differences in time and distance-related variables using the R 4.5.1 and SPSS 24 program. The main results are as follows. First, regarding factors related to out-of-hospital cardiac arrest among the study subjects, cardiac arrests caused by disease occurred most frequently, and large number of cases were transported via by ambulance and pambulance. Second, the response times in out-of-hospital cardiac arrest was significantly longer in rural areas compared to urban areas(p<0.001), and the distance to scene was also significantly longer in rural environments(p<0.001). Furthermore, Total EMS turnaround time and departure to return time showed significantly delayed patterns in rural areas compared to urban areas(p<0.001). Based on these findings, additional policy efforts are needed to effectively redistribute emergency medical resources between rural and urban areas and to train community first responders. Furthermore follow-up research is required to improve healthcare equity in vulnerable areas.
