Quantitative Detection of Periodontopathogens in Gingival Crevicular Fluid Using Real-time PCR

This study aimed to analyze the appearance rate and quantification of pathogenic bacteria based on the severity of periodontitis in Koreans and evaluate the correlations among periodontopathogens. Sixty participants (28 males and 32 females) were classified into three groups (Biofilm-Gingival Interface BGI-H=20, BGI-DL/LB=15, and BGI-DL/MB=25). Eleven periodontopathogens collected from the gingival crevicular fluid were analyzed using multiplex real-time PCR. In the red complex, which is frequently observed in the late stages of periodontal disease, P. gingivalis and T. denticola exhibited differences in appearance and quantity among the BGI-H and BGI-DL/MB (p≤.01) groups and between the BGI-DL/LB and BGI-DL/MB (p<.05) groups. Within the orange complex, which is associated with intermediate stages of periodontal disease, F. nucleatum, P. nigrescens, P. intermedia, P. micra, E. nodatum, and C. rectus exhibited differences in quantification among the BGI-H and BGI-DL/MB (p<.01) groups and between the BGI-DL/LB and BGI-DL/MB (p<.05) groups. Among the strains within the same group, the strains that exhibited significant positive correlations as a measure of influence were T. denticola from the red complex and F. nucleatum from the orange complex (p<.05). In particular, P. nigrescens from the orange complex showed a significant correlation with all strains classified in the red complex and the green complex, which forms during the early stages of periodontal disease (p<.05). The appearance rate and composition of periodontopathogens exhibited a noticeable difference based on the depth of periodontal pockets and the extent of bleeding, correlating with the severity of periodontitis. These findings can be used to prevent periodontitis, develop treatment plans based on the reduction of pathogenic bacteria, and predict treatment outcomes.
