Unified Golden Parameter Therapy Using a High-Fluence 1064-nm Picosecond Laser for Removing Tattoos Without Side Effects

Background Traditional picosecond laser tattoo removal is effective but carries a high risk of adverse effects, including permanent hypopigmentation and hypertrophic or atrophic scarring. Therefore, a safer and more effective approach for complete tattoo removal is warranted. Objectives This study was conducted to investigate the efficacy and safety of unified golden parameter therapy (UGPT) using a high-fluence 1064-nm picosecond laser for tattoo removal. Methods A total of 438 Korean patients were enrolled in this retrospective study. Participants were randomly divided into groups receiving UGPT (n = 160; spot size:7 mm; fluence: 0.75 J/cm(2); pulse duration:450 ps; pulse rate: 10 Hz) or one of five conventional treatments, including traditional 1064- and 755-nm picosecond lasers, and Q-switched (QS) 694-, 1064-, or 532-nm lasers. Treatments were repeated monthly. Results UGPT demonstrated superior efficacy, achieving complete clearance in 95% of cases (152/160). Furthermore, UGPT showed a significantly lower incidence of adverse effects (5%; 8/160), such as permanent hypopigmentation and scarring, compared to the other five laser modalities (p < 0.05). Conclusions UGPT using a high-fluence 1064-nm picosecond laser appears to be a safe and effective approach for complete tattoo removal, significantly minimizing the risk of permanent hypopigmentation and scarring compared to traditional methods.
